巻首「正五位下典薬頭安倍朝臣真貞／侍医従六位上出雲宿祢広貞等／奉勅同撰」。元奥書あり、末に「文治元年十一月／典薬頭丹波良康書／秘中之秘者誠在這両巻乎不可許他看也／権大納言定親判／重加校畢 准大臣定親」。奥書「安永二年癸巳三月 浪華木孔恭校訂」。最終丁裏左版外に刊記「大阪高麗橋壹町目 浅野弥兵衛梓行」。
